MAYSL Passcard Instructions
All previous soccer year issued passcards are not valid for the coming season. For example, all coaches must have new passcards for the Fall season. Those cards are valid for the next Spring season.
(Coaches and players use the same style card.)
STATE: Massachusetts
MEMBER: This indicates the type of member; player, coach, assistant coach, or manager. If applicable, please print the coaches license level (A, B, C, D, E, F, G) after the coach designation.
BIRTHDATE: The player's date of birth in MM/DD/YYYY format. It is not required for coaches.
TEAM: This is the name of the Club or Town.
TEAM#: If you have multiple teams in an age group use this field to uniquely identify the team name otherwise it may be left blank.
AGE GROUP: This is the age group of the team.
SEASONAL YEAR: Indicates current seasonal year (i.e. 2009).
ID. #: (leave this blank).
All players in U12 and above in divisions 1 & 2 need member pass cards.
All coaches in any age group and division (including division 3 and all U10) need a member pass card
All cards must be typed. Hand written cards will be returned to the Club unprocessed.
Only the white "MEMBER PASS" (left side) needs to be submitted to MAYS. The blue MEMBERSHIP CARD" (right side) is for the player to keep.
If the Club is entering more than one team in the same age group, each team must be identified with a unique name or number in the TEAM# field.
A recent color picture (WITHIN 2 YEARS OF AGE) must be attached to the reverse side of the white "MEMBER PASS" with glue. Staples must not be used to attach the picture to the card. The picture must not cover the player's signature. The member must sign the card in ink.
MAYS reserves the right to verify proof of age (i.e. birth certificate, baptismal certificate, driver's license) for any player member.
